About CCR at U of TWhat is the Co-Curricular Record?
The Co-Curricular Record (CCR) provides a centralized database of activities that lets students easily search for opportunities beyond the classroom. Students will be able to find opportunities that suit their lifestyle, and search by campus/faculty/college, areas of interest, or time of day. Competencies will be linked to activities, which will help students make the connection between their involvement and learning, allowing them to frame their experiences and skills to employers, graduate and/or professional programs, and for awards and scholarships. Students will be able to print at any point an official validated record of their involvement at the University of Toronto. Search. Connect. Record. The benefits of CCR?

UTSG MEDLIFE CCR (ONLY for registered University of Toronto Students)
In order to obtain an officiated CCR from the University of Toronto, participants MUST be registered with our UTSG Membership; to do so, please go here. Once a registered member, participants must complete a minimum set of tasks to get CCR accreditation.
Our UTSG MEDLIFE Chapter recognizes that not everyone can participate on an overseas trip, and thus we actively promote local volunteering opportunities. In our 2-Tiered System, volunteers can fulfill CCR requirements in two separate ways:

Please remember to complete necessary documents (i.e., Google Forms) that are required by the UTSG MEDLIFE Chapter prior to travelling with the group. In addition, you must also:

4. Complete the CCR Evaluation Form. You must receive a “Pass” from our Chapter.
In your journal, please include challenges that you have faced during your trip and how you may have over come them, describe your strengths or weaknesses were on the trip, describe general impressions and anything further you would like to include in your journal.
NOTE: Our group highly recommends participants attend seminars the same year they attended the mobile clinic trips. UTSG MEDLIFE Chapter will not validate any trips after 1 year of the date on the certificate issued by MEDLIFE (successful completion of the trip date). Therefore, it is highly recommended that the participant successfully fulfills all the criteria to obtain the CCR within 1 year of the date the certificate is issued. You can only obtain CCR if the trip was organized by the UTSG MEDLIFE Chapter.

Once completed, participants will be notified by UTSG MEDLIFE regarding successful completion of requirements. CCR will be validated by the Senior Executives of the club, and the group’s ‘validator’ will be notified.
- Participants will be asked to keep track of their records in their respective journals as well for their own personal record.

- Participate in a minimum of 25 hours of volunteer service hours with the UTSG MEDLIFE Chapter. NOTE: These hours can be accumulated throughout your degree here at the University of Toronto. Many events have bonus hours suggesting that you may receive more hours faster! Join our facebook group or membership program for more information.
- Attend a minimum of 5 seminars/events hosted by our chapter.
- Complete the CCR Evaluation form available here. You must be aware of MEDLIFE's policies and regulations and our mission. For more information, please see our instructions on the CCR Evaluation form or contact us.
CCR Requirements for Overseas Initiative
Please remember to complete necessary documents (i.e., Google Forms) that are required by the UTSG MEDLIFE Chapter prior to travelling with the group. In addition, you must also:
- Attend mandatory pre-training seminars prior to overseas travel with the group.
- Attend a MEDLIFE Service Learning Trip (SLT), participating with the UTSG MEDLIFE Chapter.
- Participant at the end of the Service Learning Trip must confirm the completion of their SLT. This is done in the CCR Evaluation Form, which you must complete once you have returned from your SLT (see point 4).

- Your journal must include a few photos of your experience at the trip.
- Your paragraph should be a minimum of 200 words. You will need to submit your response to the Chapter.
- If the participant fails to meet these requirements, they will be unable to obtain CCR.
- You may be asked to give consent if the UTSG MEDLIFE Chapter chooses to share your story to the UTSG MEDLIFE domain, including Facebook, Instagram, and our website. This is inclusive of any of the photos you submit.
